Keep Your Plan, Change Your Doctor

Switching to NorthBay Health is easy! If you are with a health plan that already contracts with us, simply call your health plan's customer service department at the number on the back of your health plan ID card and let them know what doctor you would like to change to and to the NorthBay Health medical group. You can select any NorthBay Health primary care doctor. Follow these two simple steps:

  • Step One: View the health plans that are accepted by NorthBay Health.
  • Step Two: Find the doctor that is best for you by using our Physician Search Tool by name or specialty, and view video profiles.
